Go back to your vet, the one antibiotic may not have completely gotten rid of the problem. I would ask the vet if it could be an allergic reaction to something... Then the dog tries to ease what's bothering it by licking possibly eposing the area to infection. Creating a viscous cycle first irritation then licking then infection.... Truly a question for a vet.

I know how frustrating that can be for you and your dog. My Maggie had been licking for about a year and a half. We tried everything.  I searched the web for answers and tried everything that was suggested. Maggie had licked both legs raw and bleeding and I could tell that she would try to stop when we came into the room but her legs always bothered her. I tried not to scold her because I think that it just made it worse. I would put Benadryl cream on her to stop the itching, then an antibiotic cream over that. Then I made a bunch of cotton socks to slip over her leg and covered it with a leg wrap. Everytime I tried to use a band aid she would try to chew it off so I stayed with cotton.  But the real help came when I was talking to a gal about her dog who was allergic to meat.  It caused dermatitis and itching on her dog. I switched to a vegetarian dog food with all the vitamins and minerals needed for good nutrition and in five days I could already see a difference in Maggie. Now three weeks later she has hair growing back on her legs and I hardly ever reminder her not to lick. She was also on an oral antibiotic for about three weeks. Good luck to you and your furry friend.
