My dog had something similar . The vet said it was a skin condition that he was born with. I'm assuming  his skin gets irritated, he scratches and until he  sometimes bleeds and then the cuts get pus? This could be causing the smell. That is what happened with my dog anyhow. He also lost a lot of weight and is having some trouble putting it back on. This may sound strange but iodine and Vaseline  helped him with his hair growth .

I am currently going through something similar in our dogs-case there is a big possibility that it is related to cushions disease and or thyroid problems.
