Housetraining takes time and patience.  Puppies at 16 weeks have very little control and will need to urinate at least once every hour or 2.  Puppies should be taken out every hour at least but it sounds like you are providing plenty of opportunity though by taking her out 2 times an hour.  Also make sure you take her out when she wakes up and after eating.  Feed your pup at regular times and don't overfeed.  Avoid overly salty foods.  Ensure that when she dose go out you always accompany her to the toilet area and praise her and reward the good behaviour with treats when she goes.  You can also try associating a cue word with toileting.  Until she gets better control it may be necessary for you to keep her in a safe area like the kitchen or bathroom or a room you can toilet proof with newspaper and plastic underneath.

If you crete train the pup - they learn to train their bladder - most dogs wont wee.poo in their crete. Take out crete straight to garden / area you want them to do the business....
Wait with them, praise when they do it - also give the task a command - ignore the bad behavoiur - rubbing their nose in it really wont do anything but make your dog smell.

My dogs now empty on command and is a very useful trick

What you need to do is everytime you see the dog pee rub it's nose in the pee or poop then smack it and and say you did a no no. Try smacking it with news paper to just roll it up but don't smack it to hard. I had my dog potty trained in two days doing it this way it is so easy o and don't forget to let it out side at least six times a day to. Hope this helps.

I have rubbed her nose in it, I have put her outside immediately and also put her in her cage, I have also let her know that she did something bad, I have had her since she was 8 wks old and she is now 16 wks and still pees and poops in the house. To top it off she knows that she is doing something wrongs cause she looks right at you as she does it. Please help

Smacking a dog/rubbing its nose in an accident is not going to help the issue (not to mention that it is inhumane and unsanitary). It can also lead to your dog developing a phobia of going to the bathroom in front of you. This can lead to accidents behind furniture and difficulty getting your dog to go when you are on walks.
