Has the cat given birth to any kittens or is she overdue?
If the cat shows any sign of straining/pushing but nothing is happening it's time to call in a vet. The mother cat will start getting distressed, may shake/tremble, have a temperature and/or be off her food. There may be a smelly discharge. If you think even for a moment, that there may be a dead kitten inside her, you must get her to a vet as quickly as possible, as it can be life-threatening to the mother cat too.
