It's normal. It's part of her you know what. It sticks out to make it easy for the male to hit where needed.
During heat the 2 sides of the vulva become symmetrically swollen. There should not be tissue protruding from the vulva or vagina. Your dog may have a vaginal prolapse--this is very the vaginal tissue everts and starts protruding from the vulva. This tissue is very sensitive and needs to be fixed. Have your dog examined by a veterinarian to help reduce the prolapse and clean the area.
