Parasites (those ones usually need antibiotics), internal damage or blockage, severe bacterial or viral infection like parvo, colitis or other gastrointestinal issue or something like liver or kidney trouble or poisoning are a few options. If it's the other back passage it could mean something like UTI, crystals, stones, blockage, kidney or bladder problems, internal damage again, uterine infection, or an unspayed female in season for a few of those options. It should be checked out by a vet asap in case it's serious and pretty much immediately if there are other symptoms present like lethargy/depression, unresponsiveness, fever, vomiting, the runs, just abnormal for your pet ect.
