My biggest concern is for an aural (ear) hematoma. This occurs when blood vessels break and leak blood between the skin and the cartilage of the ear. They can get to be very big in size, often feel hot, usually fill fluid-filled (like a water balloon), and are painful/uncomfortable for dogs. They are caused by trauma--either from shaking the head/ears or injury often from wrestling/playing with other dogs.
Dermal masses can develop on the ears as well. If it does not look a hematoma your veterinary will do a fine needle aspirate to determine what the growth is--cyst, benign mass, or malignant mass.
