So many different things that it can be. Mange, ringworm, allergies, and more, you need to take him to the vet to be seen and to make sure that he isn't going to get worse. Hope this helps, good luck to you.
Dogs noses can often bleed from a weed called a fox tail. It is a small weed with sliver like leaf things. It can get into their nose and make them bleed. And fox tails get into our dogs feet in between his toes all the time!! It is a huge pain but needs to be treated fast or it can cost a lot more. Of course, if you don't have fox tails...this probably is not your problem. Just trying to help. Hope your doggy gets better!
The paws don't typically become swollen from sitting or lying down on hard surfaces. This may be due to peripheral edema (fluid pooling in the extremities). It is important to find the cause.
Elbow calluses or hygromas commonly form on elbows in medium, large, and giant breed dogs--these are pressure sores that develop from laying on hard surfaces. These calluses can become infected or can disguise masses that have formed below them. The patches and bleeding from the nose are concerning. The nose is not normally traumatized from hard surfaces. They can become dry, cracked and bleed from benign or more serious causes. I would have your dog rechecked or possibly obtain a second opinion.

I just went through this with my dog. It It is your dogs immune system.
Get him to the vet right way. And have if get you the number to a dermatology/vet. And they will be able to help you.
