Not an insect - sounds more like a spider bite.
Bed bugs are making a comeback. They typically bite in two or three spots. Look for dark stains on the edges of your mattress.
Haven't a clue. But whatever it is doesn't sound too healthy. Might be worth getting it checked out if it gets any worse
