Most probably, your dog has got injury. This injury can result when dogs cross thorny bushes because scrotum can be damaged due to thorns.  This can result in bleeding. It is necessary to stop the bleeding and use some medicines to avoid infection.

You should wash the wounds and then apply topical antibiotic preparations to avoid infection. It will be difficult for your to do this. Ideally take him to vet because he knows how to handle this situation and which wound cleaning solution and antibiotic are better for your dog. So, get help from your vet.

Most probably, your dog has got injury which might be due to crossing thorny bushes. Thorny bushes can damage the pads of the paw and scrotum resulting in to bleeding from pads and testicles.

You should take your dog to vet for cleaning of wounds and prophylactic antibiotic treatment to avoid infections. Meanwhile, you should examine his pads thoroughly and look for any thorn or sharp objects.

This could be an abrasion or tissue trauma.  Dogs can also get testicular infection, testicular torsion, and testicular tumors.  Lastly there is a sexually transmitted tumor called a transmissible venereal tumor that can affect the penis (not usually the testicles) that may be a contributing factor.  Your dog is obviously painful and needs to be examined by a veterinarian.  They will help manage the primary problem and assess your dog's pain.
