It is possible that your dog might be having diabetes because increased in thirst and frequent urination are major symptoms. Dogs can also lose weight due to diabetes. If your dog is female then infection of uterus can cause increased water in take and increased urination.

   2. Infection of uterus
   3. Lead poisoning
   4. Addison disease
   5. Cushing's disease
   6. Kidney failure
   7. Liver disease
   8. High blood calcium
   9. Pituitary gland problem

So, take your dog to vet for diagnosis and treatment.
