Dogs have 2 testicles present in the scrotum. The scrotum hangs down between the legs between the prepuce (holds the penis) and anus. Male dogs also have accessory sex glands called the bulbourethral glands. The bulbourethral glands are located about midway between the scrotum and the prepuce. These glands can become swollen when dogs become excited and appear like 2 round, firm, swellings beneath the skin--these are not testicles.
Have your dog examined by a veterinarian if the possible extra testicles are located in a different area.

The above statement is incorrect. Dogs do NOT have bulbourethral glands. What the "extra set" of balls are when excited is the part of the penis called the bulbus glandis, and engorges with blood during an erection, and is the part that becomes "locked" inside of the female.
