Well the urine smell indicates an infection - take her to your vet, she will be feeling very unwell, and make sure she has plenty of water to drink at all times. Never mind the accidents - the less water she drinks the more fishy her urine will smell and the more discomfort she will be suffering.

There is water and food available for her at all times, she is never without, but will take her to the vets thanks

I am not familiar with the uraline product and it does not come up when I search the veterinary forum.  Your dog should have an urinalysis run to make sure this is not infectious in origin, screen for protein in the urine, screen for diabetes, and to check kidney function.  In any older dog that becomes ill blood work needs to be run.  Incontinence or senility can cause inappropriate urination in older dogs.  Bloodwork will also verify that the liver and kidneys can handle pain medication.
Limping can be due to sprains/strains, joint injuries, arthritis, and bony tumors.  If x-rays were not non-diagnostic and your dog is not responding to pain medication I would have her examined by a Veterinary Orthopedic Specialist--your veterinarian can refer you.

Hiya, thank you for the response, jazz had blood work done before they did xrays to ensure her liver and kidneys where working ok which all came back fine. The vets started her on Metacam but this didn't work so they gave her stronger pain killers which worked for a while but now they do nothing, they said the xrays where clear and there was no sign of arthritis, she has muscle wastage on the shoulder and won't put weight on it she just limps around, they also did urine tests before the xrays and they came back clear but then that was about a month ago so could still be an infection so will be taking her the vets to check that out, just wish they could help her with her shoulder :(

Neurological problems could lead to muscle wasting but I would have thought other neurological signs would be present. Your veterinarian can try a different NSAID (non-steroidal anti-inflammatory) and an opiate (like tramadol) for pain control. If the pain is unresponsive to medication I would be very concerned about a bony tumor--these are very painful.
If the urine was checked a month ago it is possible an infection is present especially if your dog is urinating less because she is less mobile with the shoulder pain.
Diabetes can also cause the urine to smell funny. A vulvar or vaginitis can lead to smelly urine too.
