Any idea why he started scratching in the first place as there is usually a cause? While you work on that have you tried an e-collar or anything? What does the vet think and what have you tried? Is it a breed that might be prone to syringomyelia if you are unable to find any other cause yet? Hope you figure it out and you pup is all fixed up soon.
