This could be a rectal prolapse.  You should have your dog examined by a veterinarian again since this is not resolving.  The licking will not help the problem, leads to inflammation, and can lead to infection.  You can try feeding your puppy more canned food to increase hydration in the stools and make passing stools easier.   A rectal prolapse is often sutured back in place by a veterinarian. 
Puppies are also prone to intestinal parasites.  Make sure your dog is tested for fecal parasites and dewormed appropriately.
